Many Port Moody Soccer Club forms and policies are available online in PDF format which must be viewed with Adobe's Acrobat Reader. Acrobat Reader is a free software package from Adobe which allows viewing and reliable printing of all PDF format documents. Coaching Applications:

All prospective head and assistant coaches from U6-U18 must submit a Coaching Application Form to be considered for a coaching position. The deadline for submitting these applications is January 31, 2008.
 

The Coaching Application Form is available as either an Adobe PDF document or a Word document.

If you have access to Microsoft Word, the Word version may be completed electronically. When editing the document complete all fields - you may type your full name in the signature box in lieu of an actual signature. Please remember to save the file with the Save As option and append your name to the filename (very important). The completed file may be emailed to coachdevelopment@portmoodysoccer.com.

The PDF version can not be completed electronically. Please print, complete all fields, sign and date the form and then submit it via mail to:

     Your team name goes in the top left-hand cell and the opponent's team name in the top right-hand cell.

     The referees will appreciate the players names in the same order as the ID cards. I will issue the ID cards in alphabetical order.
