Many Port Moody Soccer Club forms and policies are available online in PDF format which must be viewed with Adobe's Acrobat Reader. ByteSize Coaching:

We are pleased to have joined Tony Waiters' Byte Size Coaching program. This is an online program with a wealth of information for coaches, including practice plans, drills, player activity sheets and much more. The club has signed on for this season and we encourage you to access this site as a coaching resource. The site has information for coaches at all levels of the game. At season's end we will check our usage to determine if it's worthwhile to continue with the program in the future so please check out the program at:

All prospective head and assistant coaches from U6-U18 must submit a Coaching Application Form to be considered for a coaching position. The deadlines for submitting application forms are as follows:

Competitive Teams:            January 31st, 2012

(U11-U12 Select, U13-U18)

 

Non-Competitive Teams:     March 1st, 2012

(Micro Mini, U6-U12 House)

The Coaching Application Form is available as an editable Adobe PDF form. To open the form click here. If the form does not open, right-click here and select the Save Target As option in your browser to save the file to your hard drive. The form can then be opened with Adobe Acrobat Reader and may be completed by typing directly into the fields. Please complete all appropriate fields. Please note that you will not be able to save the completed form to your hard drive if you are using Acrobat Reader to complete the form. The form data can only be saved if you have the full Acrobat program. However, the completed form can be submitted by either of the following methods.

1) The completed form may be submitted electronically via email by clicking on the Submit by Email button at the top of the form. If submitting the form via Email please make sure you type your name in the Signature field and enter the current date in the Date field on the last page of the form. We suggest printing a copy for your records.

2) The completed form may be submitted by snail mail by clicking on the Print Form button at the top of the form. If submitting the form via hard copy please make sure you sign and date the form in the appropriate boxes. We suggest keeping one copy for your records and mailing one copy to the following address:
